Underwood Middle School Health and Physical Education

If a minor injury occurs (scrapes, mild bruises, stubbed toe, etc.), the proper treatment will be applied (Band-Aid, ice) and the nurse will not be called.  If the injury is more serious, the nurse will be called and an incident report will be filled out.  Depending on the severity of the injury, the nurse or I will contact the parents.

 

If a student is injured he or she must have a note from either a doctor or parent to not participate in physical activity that day.
